David Collyer has excellent instructions which I have used quite a bit. Maybe he'll reprint them for the list since it's been quite awhile since he did so.
I'd dearly love to but I don't think it was me!! I certainly received one of those doodads years ago from Stormy Lee but can't recall ever having had the instructions to make one.
