Yes. You clamp it to the edge of a table or something similar as show in the left hand picture. You could either wind your thread/yarn onto the spool provided from a skein and reel the thread off from it as you needed it so that it came off straight and did not twist or untwist as it would if you pulled the thread off from the top of a spool. Or you could place one of your own spools on the spindle in place of the one provided.

It's similar to the wooden spool holders we can still buy and pin to our pillows so that thread is pulled off without twisting and untwisting.

The Victorians did things with much more style than we did.
