The book "Milanese Lace: Original Patterns in Color" has most patterns in three sizes, for different threads. There's detailed starting information, the listing of each braid used (which can be found in the basic braid books most people have), and hints about special situations you might find. The back of the book has some basic technique details. Van Sciver Bobbin Lace lists the book for $19.95.
