In her very useful description of pillows, Alice mentioned the "One and Only" from Snowgoose, and added,

Snowgoose sells the forms only.  The pieces will need
to be covered and assembled.  However, Lacy Susan has
the same kits already covered.  There could be other
dealers with this or similar pillows.

I'm very sorry to report that Lacy Susan no longer carries these pillows covered. As you can imagine (and as I know from covering another block pillow), this is very labor intensive (although worth it). Her business - and her life - just don't leave enough time to cover pillows any more. I'm glad I got one while she was still doing it - she did a wonderful job.

One of the nicest things about the One and Only is that they are relatively inexpensive. When I went to IOLI in Denver last summer, I needed to be able to check my pillow. I bought a O&O kit from Snow Goose and cut enough off the end of the pillow (which would be farther away from me when working) so that the pillow would fit into my large checked luggage. It's nice to have an extra O&O - and to be honest, I never got around to covering the blocks in the one I covered - I just use the ones out of the Lacy Susan pillow. One of these day, I'll "get around to it".
