The big deal with a surgeon's knot is that the first half of the knot doesn't 
loosen while you are tying the second half.  When bundling old papers for the 
recycling center, I generally keep wrapping until I have about an inch of 
twisted string, then pull the string tight, put my finger on the twist to keep 
the string from slipping back, and make the second half of the knot.  Then I 
tie a third half to complete a square knot on top of the twists, since the knot 
can't close properly on that many twists.
