Thank you all for your insights regarding this ground stitch - it's a treasure trove of lace-making experience here! You were right in that the photo of the lace was turned 90° compared to the pattern. The dragon is worked left to right and then it all makes sense.

Kat stitch is shown like that in the Cook/Stott book, also the similar Point de Mariage. But in Nottingham's book "The Technique of Bobbin Lace" p. 136 it's done with pins under the stitches like for the Paris ground (which makes Kat stitch and Paris ground the exact same thing). But Cook/Stott say that the pins should be taken out as soon as possible, so that the holes disappear - which will probably look the same in the end as putting the pins below the stitch from the start.
