I think there is a knob at the other end, too, - deep in the shadow of the
basket.
I first thought it was for winding up your yardage lace, but the knobby bit
in the centre would make this difficult. Then I decided it might be where
you can wind on threads - like some of the DMC thread that comes in a skein,
not a ball. When they fall apart the thread tangles, so winding it on a core
of some sort eliminated this.  There are 2 necks so 2 threads could be held
on the one tool.

Still, - a needle  case - yes, that seems probable if the end takes off.
