My solutions to these problems have been super-thrifty.  Although highly
nonstandard, I'm sure, they work for me.  

I have used a small table intended to support a laptop computer as my
"cookie pillow stand."  The table surface (17-1/2" wide x 15-3/4" deep) can
be adjusted between flat or tilted, and the height is adjustable.  There is
a "lip" across the front of the table surface that helps to keep the pillow
from slipping off--and I topped my table with a layer of rubberized
shelf-lining, which does a good job of keeping the pillow on the table
(thankfully, no accidents so far).  I caught a sale at the beginning of the
university's term on the laptop stand and paid only $15 at WalMart (current
price at the link below is $19):

I sometimes use a "pregnancy pillow" to rest between my knees and the bottom
of the cookie pillow when I want to sit in my reclining chair and make lace.

I am very much a beginning lacemaker, so it is possible that I will decide
to "upgrade" at some point in the future.  But these items seem fine to me
now (perhaps a case of "ignorance is bliss?")
