Hello All! Many thanks to all who have written with help & suggestions. I posted a pic of the page in question on Arachne Flickr & Sue B has kindly made it into a JPEG. If anyone has a copy of the IOLI charter chapter cd from 2008 & would be willing to look for it, we would be most grateful.
