For a librarian, I'm not the most organized when it comes to my prickings. I do have them labeled with the pattern source, thread used and most have number of pairs used. Most are stored flat in a pretty box I found (at the National Library of Australia Gift shop, I think), and some have been stored in a file folder. The only prickings not in the folder or box are those that are in my Stage 1 proficiency test workbook.
I do have a notebook that I have all of my working notes and accompanying thread samples of each piece I make. My documentation habits got ingrained whilst doing my ALG proficiency test, and I kept it going for the day I do the next stage.
