I've installed the 5.0 version of LAM Pro and, for some reason, now I 
can't connect to the self-service page.  It gives the error "Cannot 
connect to specified LDAP server. Please try again."

The logfile only shows this:

2015-07-24 11:33:18: LDAP Account Manager (7aphljuetqm2ncqgfl6dfea9l3 - 
xx.xx.xx.xx) - ERROR: Unable to connect to ldap://hostname.domain:389.

Funny thing is, on the regular /lam/ page, it connect to the ldap server 
and I can login (using the same account credentials) just fine.  Any 
ideas as to what might cause this?  As far as I can tell, the server 
settings are the same for both areas of LAM.  I have screenshots of the 
server settings if that helps...
