Details:
LzInstantiator, LzNode: Use a separate name for creating the
artificial profiler call trees in the instantiator.
kernel.js, LzRuntime: Be more careful about poking at globals
LzNode: Accept _profile_name from Replicator. Add event for
destroy, since there will not be a done event in that case.
LzReplicationManager: Pass annotated profile name to clones.
CodeGenerator: Improve pseudo-tick accuracy.
