Could you add this information to a more public place? That'd be in the
wiki or i.e. in "tools/trunk/svn/README.txt".
Because it is quite hard to follow a standard, if this standard was just
defined in a single mail, especially for new committers (like me!).

Run svn log -v -r<fix number> and paste the output into the comment.
Belt and suspenders. I know there is a hook that is supposed to find
relevant svn check-ins from Jira, but I like the double-entry
bookkeeping -- it will help us track things down if there is any
disagreement.
Also, please update the Fixed in Change# field with your svn <fix
number>.
