Summary: Fix braino in html-response xslt templates
Bugs Fixed: LPP-6496 - html and app_console request types should
have the same XSLT structure

Details: html-response* - add missing <xsl:otherwise> after pocketpc test.
app_console* - Add a link to show only the application with lzt=html
Tests: Apps load as before with and without lzt=html
