Summary: Clarify documentation on embedding inside existing divs
Bugs Fixed: LPP-8678 - embed.swf put appllication in wrong place, embed.dhtml
show an incorrect warning but works

Details: Clarify that divs should appear before any calls to
lz.embed.dhtml/swf() in order to be detected for insertion/appending.
Tests: See deploy: Server button for any app.
