There is a text file called "tiers.txt" in the directory /docs/src You can create a new tier by making an entry in that file.
Similarly, there is a text file called "categories.txt" that defines the allowable categories used to build the table of contents for the reference.
As Jim said, please note that these tools are somewhat tempermental. They have not really been factored or engineered for creating user docmentation, although that's certainly something we would like to do at some point. They are powerful tools but not especially easy to use, and it's easy to be confused by them. (Or, in any event, it's easy for *me* to get confused.)
