You haven't installed the files in latex2html/texinputs where
latex will find them.

Also ... this is not your immediate worry, but your version of
ghostscript is 2.6.2 ... you really should upgrade ... check out
http://www.cs.wisc.edu/~ghost/index.html for Aladdin's version 5.50
which is excellent.
