This fixed the problem in the chapter headings, but not in the section
headings.

These fixes didn't seem to get into the latest developer's version.
Also, these fixes repair the problem above, but they make single
line verbatim environments that start with a percent, like this
\begin{verbatim}
% Line
\end{verbatim}
disappear in the output.
