If the variable $DVIPSOPT in your l2hconf file includes the
option "-Ppdf", such a problem may occur. Please remove "-Ppdf".
When using -Ppdf, dvips will load alt-rule.pro. But, the underline
will remain on latex2html for the image made by dvips +
alt-rule.pro.
In some TeX packages, dvips -Ppdf loads alt-rule.pro and
overrides it by the followed file tex.pro or texc.pro. So, such a
behavior may change by the version up of TeX package.
