500 Internal Server Error

I'm trying to write a text in an image and show in the browser. This
code[1] not works (returns error 500).

Look into your apache logs for more details.

Where do you load the font path ?
I miss things like

  ftFont.InitEngine;
  FontMgr.SearchPath:='/usr/lib/X11/fonts/truetype';

or on windows

  FontMgr.SearchPath:='c\windows\fonts';

  f.FPColor := colRed;
  c.Font := f;
  c.TextOut(10, 20, 'sample text 12345');
  img.SaveToFile('test.png');

Has the cgi script write permissions when running as apache user?

It is on windows, so it should be OK.
