What you can't do without "interfaces" is (1) use a local GUI (such as GTK or QT) and (2) do the usual "event driven programming" (e.g. use TTimer and anything based on "normal" inter-Thread notification).
So without "interfaces" your project is not an "Application" but a "Program" (AKA "command line tool") that is started, does its work, and stops; without waiting for anything to happen while it runs.
I suppose by "real Program" you mean "Application" (according to Lazarus-speak), so you can't use noGUI.
