Are you sure that the colors used in your images are within the gamut of your input profile? Maybe they are out of range in the first place.

Yes, the profile seems to claim so. According to the gamut LUT recorded in the USWebCoatedSWOP.icc profile, the color CMYK=[100,100,0,0], which corresponds to PCS L*a*b*=[30.919118 19.988281 -48.363281], _is_ out-of-gamut.
But CMYK=[100,100,0,0] is indeed a border case. Basically it should be reproducible, since I assume that the SWOP total ink limit is by far > 200%, but since all four components are at their high/low limts, it is a color at the gamut boundary, so it's neither clearly inside nor outside - and the gamut LUT has only a limited accuracy.
