As for the PHP problem, I'd guess there's something in your ACL
configuration it is unhappy with. But PHP's LDAP support is not all that
great to begin with.

My guess is that slapd is missing the schema file that defines
shadowLastChange. What does running slapd -d -1 when it appears in
