I'm not sure what tool to use to turn a CSV into a good-looking calendar, but I do know what I'd use for general PDF creation in Windows, and it's free: http://www.pdfforge.org/products/pdfcreator/ Actually, I do know exactly what I'd use for making a good calendar, but it doesn't start from CSV (it uses a plain text file with a list of events).
