I think a problem is the fact that EXTERN_TCP_PORT0 is commented out.
You need to have all variables defined in order starting with zero.

> EXTERN_TCP_PORT2="0/0 3389"
> EXTERN_TCP_PORT3="0/0 22"
> EXTERN_TCP_PROT4="0/0 85"
