Not sure if it will help, but I compiled lspci against
glibc-2.0.7 a few weeks ago .  The binary is at

  http://fritzfam.com/brad/leaftmp/

If you grab lspci and pci.ids from that directory and run

  ./lspci -i pci.ids

you can display which NICs are recognized on the PCI bus.
(I think the -v option might even list the IO ports.) If
the fourth is not listed, the problem is probably in the
BIOS config, or due to some characteristic of the mobo or
the way the cards are installed in it.
