        Using Syslinux 1.67 or above with any version of M$DOS other than M$DOS
6.22 can cause issues.  I have tested every version of Syslinux with
every version of M$DOS and different hardware configuration with dim
results on IDE devices.

        Grab a image of M$DOS from my website http://leaf.netvantix.com/images,
if you are using Windoz grab WinImage from http://www.winimage.com and
the .imz file.  If you are using Linux grab the .bin file for use with
dd.

Make the image and then follow the steps as before making sure to remove
all partitions from the hard disk prior to recreating a new one.

Make sure the partition does not exceed 20MB ( yes I know the syslinux
limit is 25MB but just to be safe ).

Then reboot after each step.

You can omit the lock step when using M$DOS 6.22

If you are using Bering be sure to use a newer version of Syslinux 1.67
or above.

If you are using Dachstein use version 1.45.

        If using Bering Make sure you have added the ide-* entries to the
LINUXRC files.  Per the docs at
http://leaf.sourceforge.net/devel/jnilo/bubooting.html#AEN1139 

        If using Dachstein be sure you have the ide capable kernel installed. 
Grab the one off of the DCD if you need it.
