Try this:
$ ledger bal -T '(l==0)?O:a'
The -T changes the value expression to calculate the total. The
conditional expression, (l==0), checks the account depth---if it equals
zero, it displays the regular total. Else, it displays only the
individual account balance, a.

of the manual.
