Hmm.   Looks like the use of initiate.pl was not documented in the INSTALL
file.

The easiest way to set this up is using Sadash's contributed interface for
database setup.  Direct your browser to
http://[lsmb-url-dir]/initiate.pland enter your database superuser
credentials.  Then initialize a database
on that screen.  That works pretty well, actually.
