[LU] Non-LU: What should the ref do?

Someone sent me this.  I think it's pretty interesting, but I'm a bit stumped on the exact answers.  Anyone have any ideas?     1. The ball is played into the box and the goalkeeper, under pressure from an attacker, makes a great save and falls into the net, the ball stays in play. The forward also ends up over the line inside the net. The goalkeeper then takes exception and whilst the 2 players are behind the goal line in the net, the goalkeeper punches the attacker. The referee sees this and blows the whistle. What action does he take and how is the play restarted?   2. An attacker goes past the goalkeeper and kicks the ball towards the open goal. The goalkeeper throws his boot which strikes the ball and prevents it entering the goal. What does the ref do?   3. A player, other than the goalkeeper, standing in his own penalty area, holding a shin guard, hits the ball with the shin guard to prevent it entering the goal. What does the ref do?   4. Player number 12 from City scores a goal however after you have restarted play with a kickoff, you realise that a substitution has been made where City number 12 came on as a substitute without your permission. What action do you take?           -brian
