I redid the report this time using end notes and they came out fine.
However, there was no Name Index generated at all this time even though I
told it to.
Very interesting indeed!

fine. The first 5 entries are correct. Then it is interupted with the
"Name Index" heading but it is really the rest of the source list BUT they
now have roman numerals instead of cardinal numbers in front of them.
Very interesting indeed. I am going to play around with the output
settings
to see what else happens.
