Just like any index in your word processing program.  The locations
are tagged and you just have to create the index once you have the RTF
file open in your word processor.

I can't be more specific with details because the word processing
programs all vary.

At this time, you can create either the Name Index or Location Index
in RTF, but not both.

PDF will create both indexes.
