There is a known problem with the Forms Center for some users and it's in the tracking system for the programmers to work on.
We don't know why it affects only some users and we thought we saw a pattern but it's not consistent. Please be patient as the programmers work on this issue. If you want to send this in to us on the Problem Report Form, we can investigate this further with you and add you as a reporter of the issue.
