I do enter the website in the proper places on the template - but for
any other "title" the only place I enter it is in the source index title.

For instance, I have started doing some of my 1940 Colorado families
since FamilySearch has now gotten those indexed and searchable.  My
title within the master source list would be:

Census. 1940. U.S. Colorado, Denver County (FS)  - In this case FS
stands for FamilySearch.

My actual citation - using only the template is:

1940 U.S. census, Denver, Colorado, population schedule, Tract 21,
Denver, Election District F, enumeration district (ED) 16-76, sheet 63A,
p. 495 stamped, dwelling 438, family 68, Noel B. Harris; digital images,
/FamilySearch/ (http://www.familysearch.com : accessed 28 Apr 2012);
citing National Archives and Records Administration microfilm T627, roll
485.

Any subsequent citations would look like this:

1940 U.S. census, Denver, Colorado, pop. sch., Tract 21, Denver,
Election District F, ED 16-76, sheet 63A, p. 495 stamped, dwell. 438,
fam. 68, Noel B. Harris.
