There are two problems with that template. 1. If you notice the hint text in all the fields are incorrectly sequenced. The hint text for each box refers to the line above and line one Jurisdiction State has hint text "Type the name of the person..."
2. The Repository City box cannot be edited, it takes the city from the address of the Repository in the line above. I have entered a problem report to have these errors fixed.
