Try the Name List in Legacy. You can do a search first to narrow it down or just use the whole name list. Use the Print option to choose the columns you want showing. If you want this on a map you'll need lat/long which is available there for all the vital records. So add the ones you want to your columns.
Then Create as csv. You need that to import (comma delimited) into Google Fusion Tables.
