You should update to 7.5.0.208, that is the most recent version. Go to Options > Customize > Data Entry There is a setting there to add a default surname when entering a child that may have turned off.
By the way, the surname will only be automatically added if there is a father. If the father's name is unknown or has not been entered yet the woman's surname will not be added automatically.
