Personally, I only use an e-mail template if someone has sent me some unsourced information. In a case like this I would rather use a suitable template for an online Church record. I don't know the site you mention, but I presume you saw all the details above, or an image of the Baptism Register, on screen before applying for the transcript? If that is the case, I would use one of the Church Records templates - Online database or Online images, whichever is appropriate.
