Hi Sydney, Do you mean the Questionnaire form in the Reports menu? Are you using the default settings, or have you changed any?
Which version, exactly, of Legacy v8 are you running? Look in the Home tab to find out, should be 8.0.0.xxx - it's the 'xxx' that we need to know.
