The default format is city, county, state, country. If you have something below the city level, as in a cemetery name, I find it best to format it as city (cemetery), county, state, country. Others use a dash instead of the (). For sort order, just use right to left. If you are going to use the 5 positions, you can still use the right to left sort. If you really want the 5 positions and the sort to be State, County, City, Place, Country, then probably best to select Generic Terms (vs US) and sort order Position 4, Position 3, Position 2, Position 1, Position 5. If you use the 5 positions as you currently are, you will not be able to use the Expand/Contract functions. Expand/contract is dependent on State being in Position 3 and country (if present) in Position 4.
