It could be that the media files were referenced in multiple places. I came across a situation today in 7.5, where I'd renamed an image file. Checking revealed one reference, so I changed that one and ran the error report again and still had one reference to a missing image. So, worth running the report until there are no errors! Is it possible that there is a bug here?
