They can't. Gedcoms are text files and can't include graphics.

You can do a multimedia backup and hope that the recipient's genealogy
program will be able to link the pictures from the picture links in the
gedcom.

If you're sending the file to someone who's using Legacy, you'd want to
back up the data and media files and send that.
