It's the relationship that is being called "step", not the 3rd great-grandmother.
It does seem a strange way to express the non-relationship. Strictly her first husband is not a step 3rd great-grandfather and so his father is not a step 4th great grandfather but it's a way of indicating that this isn't a blood relationship. If it just said 3rd great-grandmother's husband's father, would you be left wondering whether it was a blood relationship? So how would you describe the connection?
