My question: Since the format for D/C's has changed over the years and
the available information is different, it is difficult to come up with
a format or template for the information. There does not seem to be any
reason to duplicate what is on the abstract and what appears on the D/C.
So, what is the best way to record the information on the D/C in the
Notes / Medical section?
I only record the actual cause of death in the Medical Notes section -
copied from the DC. I included in the Notes any additional information
such as "Certified by Joe Bloggs MD" where available.
If someone else in my database is named as the Informant I create an
Informant Event for that person.
The complete entry on the DC I only enter as part of the Source
Citation. (The Source, of course, may be used for many different
fields, such as date and place of death, possibly birthdate, possibly
parents names, etc.) I attach a scan or downloaded image of the DC and
copy ALL the information into the Source Detail Text field. To make
this quicker and easier and consistent I have created a list of the
headings on a DC and saved this to M8 Free Clipboard for calling up when
required. (I have these templates for all frequently-used documents and
images.)
Also, the D/C's came from many different online locations, some of which
may not be currently available.
How would I source a D/C downloaded online when the original web page is
gone?
All websites are liable to change, change of location or total
removal. As it is something one has no control over I think we just have
to live with it and move on. My Source tells people where found the
information and helps to give it authenticity. The full text and an
image where available are included so if a future researcher cannot
access the same webpage they are not losing anything.
