Theresa,
You can have the Surname first if the cursor is in the Surname search
box when you are generating the CSV
OR
Don't include the name from the first section but add Surname and Given
Names separately in the Customise section (or as well if you want
married names as the married surname is only returned by that first
section where you can only choose whether or not to include them).
