In legacy 7 it is Report, Books and Other, Relationships
Legacy 9, Reports menu, Other Reports select Relationships from dropdown menu

It would need to be the "primary" relationship as shown on the Family view screen, not any of the others shown in the Relationship Calculator.
By "form" the list I mean by focus group, or tag, or hashtag or anything else.
