I contacted support and got the following response, which fixed the problem:

Go into your file and run a file>file maintenance>list cleanup and run the
list cleanup tool.  Next run a file>file maintenance>rebuild source
citation.

After the list cleanup and rebuild if the problem still occurs try
exporting to a new file.  To do this open Legacy and your file and go to
file>export>Legacy file and export to a new Legacy file.
