One VERY important issue with generating any kind of tables or indices in MS
Word is that the Show-Hide button MUST be turned off to generate correctly.
If you can see hidden codes, paragraph marks, etc. the button is ON.  To
turn it off, click on the paragraph symbol on your toolbar.  If the feature
is turned on when you generate, the page numbers, etc. will *not* be
correct.
