Hi Lincoln I've once tried to find programs that can convert pdf files to text files. They exist, but most of them had very poor results. Don't know their names anymore but they can searched (www.google.com), I even think Adobe has a plug in (pdf2txt?).
I think typing your own questionnaire in your word-processor would save you a lot of time, another option could be printing the blank form and scan and ocr it afterwards.
