You could deal with this in a couple of ways (at least).
It is a primary source document enter it in the master source list, with
date, etc included.
And/or
Make it an event and create your own sentence to go with it (in Deluxe),
source it back to the source document, above.
Or
Put it in the birth notes - use the cross at the right of the birth
details line, sourcing a/a.

What ever you do - test out the result by checking on several reports to
see what it looks like.
